Una operación militar en Octubre de 1993 guió a un conflicto urbano de alta intensidad en la
capital de Somalia y aportó contribuciones relevantes a la investigación en el área de cirugía de
trauma. Las conclusiones sobre la conflagración definieron un nuevo enfoque para la atención
de las heridas, el trauma urbano moderno. Cuestionaron la resucitación agresiva con soluciones
cristaloides, identificaron el retardo en el traslado, recomendaron el uso precoz de antibióticos,
rediseñaron las armaduras corporales y reafirmaron el uso de los torniquetes. La hipotermia y la
coagulopatía fueron reconocidas como factores adversos. Representaron un importante cambio
en el manejo contemporáneo de los pacientes lesionados. El personal medico requiere la
adecuada comprensión y preparación para afrontar este tipo de lesiones. Sucesos violentos en
Caracas y la reciente pandemia por la COVID-19 determinan una nueva orientación en la
atención de las heridas.

The black hawk downd and the new management of the patient with injuries
Abstract
A military operation in October of 1993 led a high intensity urban warfare in the capital of Somalia, although short in time and no many troops, contributed in relevant investigations in the field of surgery of trauma. The conclusions of the conflict determined a new concept for the attention of the patient with injuries, the modern urban trauma. Argued about aggressive crystalloid resuscitation, identified the delayed in the evacuation, recommended the prehospital prophylactic antibiotics, the utility of tourniquets and including body armoury design. The hypothermia and coagulophaty were recognized like adverse factors. Represent an important change in the management contemporary of the patients with injuries. The surgical personal need understand this new kind of injuries. A resents events in Caracas and the new disease COVID-19 represent a challenge in the attention of these patients in the emergencies rooms.
